Home and Community Care Program for Younger People
HACC / PYP
The Program for Younger People (PYP) provides support for Victorians under 65 who have a disability, chronic illness, or other care needs and are at risk of entering residential aged care. Funded by the Victorian Government, PYP offers tailored services such as personal care, respite, case management, and home modifications to help individuals live safely and independently in their own homes.
PYP aims to bridge the gap for those not eligible for the NDIS, ensuring access to essential care and support while promoting dignity and community inclusion.
Commonwealth Home Support Program
CHSP
The Commonwealth Home Support Programme (CHSP) helps older Australians stay independent and connected in their communities by providing entry-level support services. These include help with household chores, personal care, meals, transport, and social activities. CHSP is designed for people who need a little extra assistance to continue living safely at home.
It’s a flexible, government-funded program that works with individuals to choose the services that suit their lifestyle and needs, promoting wellbeing and independence.
Support at home
Previously home care packages
Support at Home is a government-funded program designed to help older Australians live independently and safely in their own homes for as long as possible. It provides tailored services such as personal care, domestic assistance, transport, social support, and home maintenance. The program is part of the aged care reform initiative and will eventually replace the Commonwealth Home Support Programme (CHSP) and Home Care Packages (HCP), streamlining access to care and improving flexibility for individuals.
Whether you’re looking for help with daily tasks or more complex care needs, Support at Home aims to provide the right support, at the right time, in the comfort of your own home.
HOW WE CAN HELP
The Home and Community Care programs are designed to enable the elderly, the disabled and isolated to maintain independence in their own homes.
Cleaning
Support workers assist in the cleaning of your home and maintaining that standard with each visit. Vacuum and mop all accessible floors, wipe and dust benches and surfaces, clean mirrors, sinks and taps, clean tiles, shower screens, clean and sanitise the toilet, dust furniture, dust TV and appliances, empty visible garbage bins.
Domestic assistance
Domestic Assistance supports older people and those with care needs to live independently by helping with everyday household tasks. Services may include cleaning, laundry, dishwashing, and bed-making. This practical support ensures a safe, hygienic home environment and reduces the physical strain of maintaining a household, promoting wellbeing and independence.
Personal Care
A Support Worker can assist you with bathing and showering, going to the toilet, offer support with shaving, brushing teeth, dressing, and getting you ready for the day, medication reminders, meal preparation, and getting ready for bed. Dhauwurd-Wurrung will work with you to find a Support Worker who matches your preferences and helps meet your needs.
Community
We will work with you to achieve your goals through accessing Community and participating in social activities. A Support Worker can assist you to get to appointments or attend activities that you enjoy. Depending on your goals and your needs, a Support Worker might assist in re-engaging with Community, in assisted or unassisted shopping.
Home Maintenance
DWECH’s Maintenance Crew services include home repairs like changing light bulbs, checking smoke alarms, hanging pictures, small repairs, making pathways safe, and cleaning gutters (depending upon height). DWECH will work with you to determine what services can be provided in line with your plan and your goals.
Gardening
Gardening services assist individuals who are unable to manage outdoor tasks due to age, disability, or health conditions. Support includes lawn mowing, pruning, weeding, and general garden maintenance. These services help maintain a safe, tidy, and accessible outdoor area, enhancing wellbeing and allowing people to enjoy their home environment.

If you have a complaint, this may be done by talking to the Home and Community Care Manager at any time. You can also write your concerns on a complaint form (someone will be available to help you fill this out if needed); or over the phone to (03) 5521 7535.
